Please print this out to take to the pediatrician. We will need this document filled out and turned in before your child can attend our center. Thank you!

Please fill out this form if you are bringing in medication for your child to receive at the center. We are required by law to follow these regulations. Carolyn Hardison and Kristi Bowles have completed the Virginia Department of Social Services (VDSS) approved Medication Administration Training (MAT) course. Carolyn and Kristi are the only staff members who are qualified to administer medication at WGCLC.

Here are the guidelines WGCLC must follow:

1. One Medication Consent Form must be completed for EACH medication. A note or phone call from a parent can not be accepted. Please be aware that this is a lengthy 2-sided form that requires more time to complete.

2. The Medication Consent Form completed by a parent is only valid for medications administered for 10 days or less.

3. The child's health care provider MUST complete the Medication Consent Form for long-term medications (more than 10 days) OR when the dosage directions state "consult a physician." (for example, Benadryl)

4. All medication must be in its original container with the child's first and last name on it.
